Union Meeting House Society of Cooks Prairie
To all whom it may concern
We the undersigned do hereby certify that on the 20th day of April A.D. 1847 certain citizens of the Township of Clarendon & Eckford did assemble for the purpose of organizing a society to build and keep in ____ a house for public worship and the undersigned have been elected a Board of Trustees of said society.
Now therefore the undersigned and our successors in office are forever hereafter to be called and known as the Board of Trustees of the Union Meeting House Society of Cooks Prairie.
Signed and sealed this 6th day of May A.D. 1847.

On this 6th day of May in the year of our Lord one thousand eight hundred and forty seven personally came the above named Elijah Cook, Elonzo H. Rogers, Ira Sumner, Ira J. Wilkins, and Charles Doolittle known to me to be the persons who executed the above instrument and the severally acknowledged the same to be their free act and deed.
